2. If ordering by Email or Fax, include which items you would like to order and how many of each you require.
(Tip: If ordering from one of our catalogues, you can print off the order form provided and fax it in OR Email your order using the item codes listed)

3. Please do not forget to include:
- A contact name & company name (ie school or church)
- Billing Address
- Shipping Address (if different from billing)
- If you would like to pay by credit card or have us bill you
- Phone number (in case we need to get ahold of you)
